Capital Counties story (so far) along with a few other Community Health Assessment colleages at the 2011 Open Forum hosted by the National Network of Public Health Institute on December 12th.  This effort, funded by the Robert Wood Johnson Foundation, is bringing together public health practitioners as part of the "Community of Practice for Public Health Improvement". See the presentation here. The Healthy! Capital Counties portion is the second of three communities highlighted.
