Do you live, work, or play in Clinton, Eaton, or Ingham counties? If so, you are invited to the 2nd Stakeholder Input Meeting for the 2018 Healthy! Capital Counties Community Health Needs Assessment! Community leaders, health care providers, community organizations and coalitions, and anyone interested in improving health can attend! At this event we will be working with the Tri-County Regional Planning Commission to map available assets in our community, gathering input from attendees on questions for focus groups and community surveys, and providing additional updates on the assessment process!

The November 2017 update to the Eaton County CHIP is now available. This update shows the progress made by participating agencies and coalitions towards the Community Health Improvement Plan goals through October 2017. Great work Eaton County!

Now that the 2015 H!CC Community Health Profile and Assessment is completed, it's time to take the information from the report and decide how we can take action to improve health in the Capital Area! The attached document outlines the vision of the area health departments in making action happen in a meaningful and coordinated way!

After more than a year of work gathering data, conducting focus groups, surveying community members and healthcare providers, and generating maps -- the 2015 Community Health Profile and Health Needs Assessment is here!  Click on the tab that says "2015 Report" above.  

Interested in a presentation of the new data, or in more focused assistance on using this data in your organization or group? E-mail us! What are the most important health issues in our area?

Here is your chance to give YOUR OPINION on health! If you are an Clinton, Eaton, or Ingham County resident, please take our online survey, CLICK HERE.

The Healthy! Capital Counties (HCC) project includes area hospitals, health departments, and other partners that are assessing the health needs of the residents of Clinton, Eaton and Ingham counties...and we need everyone’s voice to be heard! Please share and post to websites and Facebook Pages, and encourage EVERYONE to take it.

The survey is a brief, anonymous, online survey intended to understand the health needs of the community and where change can be made for health improvement. Thank you for helping bring better health to our communities!
